Heights and Hills Communities Served

WHO WE SERVE

Each year, Heights and Hills serves close to 3,000 adults over the age of 60 and their families living in Brooklyn. Many of our clients suffer from multiple chronic conditions. For those who have friends or family involved in their care, we provide information, direction and support to enable the family to care for their loved one. However, many of our clients live alone and are homebound and isolated without friends or family to help them.

We provide case management and caregiver services to the following neighborhoods:

  • Carroll Gardens
  • Clinton Hill
  • Cobble Hill
  • Crown Heights
  • East Flatbush
  • Farragut
  • Fort Greene
  • Gowanus
  • Park Slope
  • Prospect Heights
  • Prospect Lefferts Gardens
  • Red Hook
  • Remsen Village
  • Sunset Park
  • Windsor Terrace
  • Wingate

We also provide caregiver services in Bedford Stuyvesant, in addition to all of the above neighborhoods.

Services are provided to Brooklynites over the age of 60, without regard to race, religion, national origin, gender, disability, marital status, sexual orientation or economic status.
