Case Management
Through home visits, professional social workers meet with older people and their families to help them cope with the stresses of deteriorating health, the threat of being alone, and the difficulties of managing everyday life. Social workers help families access home care, entitlements and benefits, counseling, home delivered meals, as well as end-of-life planning.

Meals on Heels
Each day, a diverse group of dedicated volunteers deliver more than 40 meals (on foot) in the Brooklyn Heights neighborhood to seniors who are unable to prepare meals for themselves. In Boerum Hill and Cobble Hill, meals can be arranged through a local Meals-on-Wheels provider agency.

Transportation Services
Our Senior Shuttle van provides fixed-route transportation to seniors of Brooklyn Heights, Concord Village, Cobble Hill and Carroll Gardens. Making four continuous loops daily, the Shuttle provides seniors with transportation to shopping, doctor visits and social activities. Our second van makes scheduled group trips to senior centers and shopping areas. We also provide door-to-door transportation for medical appointments.

Nursing Center
Heights and Hill cosponsors The Nursing Center in partnership with SUNY Health Science Center at Brooklyn’s College of Nursing. Working closely with our social workers and with SUNY faculty, student nurses provide examinations and health promotion to seniors at our site and for the homebound.

Caregiver Services
Heights and Hill sponsors educational workshops and support groups for families who are caring for aging family members. Workshops are taught by experts and provide family members with the tools they need to assist loved ones.

Thanksgiving Dinner
Each year, Heights and Hill coordinates a community dinner on Thanksgiving Day at Grace Church in Brooklyn Heights. The dinner is a true community endeavor, serving approximately 200 older guests who would otherwise spend the holiday alone.

Education/Cultural Programming
Heights and Hill provides weekly activities to senior residents of a large apartment complex in our community, as well as parties and special events.
